Last Action column in the My Workflow Assignments page is NOT Populating (Doc ID 2508969.1)

ISSUE

After upgrading to WebCenter Content 12.2.1.3.0 the Last Action column in the My Workflow Assignments page is not displaying data when updates to a content item have been made. Previously would see the words "Review", "Update", "Check Out", "Undo Check Out", etc. Now it doesn't display anything, occasionally will see "Timed Update".

Last Update Column does not display anything.


STEPS TO REPLICATE
-----------------------
1. Create a Criteria-based workflow.
2. Check-in a content item with criteria matching above workflow.
3. Go to content information page and verify that the status is - Review.
4. Navigate to My Content Server -> My Workflow Assignments page and see that the content is in workflow queue. Notice that the ‘Last Action’ column is not populated.
5. However, when navigated to Workflow Content Items page, it is in Review status.
6. Reject the content item.
7. Now the Last Action column displays 'Reject'.
8. The Workflow Content Items page shows it in Edit status.

Changes

Instance recently upgraded to 12.2.1.3.0 but this may also occur on new installations as well.
