OHS 12c Fails to Start with Syntax Error in httpd.conf After OAM WebGate Configured

Symptoms
After installing Oracle HTTP Server (OHS) 12c and configuring the Oracle Access Manager (OAM) WebGate, the OHS instance fails to start with a syntax error, similar to the following:

The file in the failure could also be webgate.cgi rather than webgate.dll, if the environment is Linux or Solaris.
