In EM Console Adapter Properties Couldn't Be Updated With PolicyApplication Mbean Error (Doc ID 2626622.1)

Symptoms

There are different SOA Composites with inbound adapters (File, FTP, etc) and when it is checked the information in EM Console or updated their properties appears the following exception:
[java.lang.Exception: PolicyApplication mbean 'soa-infra' does not return expected effective policy sets.]

for example of AQ adapter, it could occur with these steps, even Lazy Loading feature is disabled:
1. log into enterprise manager.
2. click on the AQ adapter of a SOA composite.
3. go to properties tab, and change queue name value to an invalid value.
4. click on 'Apply' button.
5. after a while, the page displayed 'Cannot find Policy Manager MBeans. Either Policy manager service is down or Policy Manager MBeans have not been deployed properly. Please contact System Administrator for help'.
