Oracle Adaptive Access Manager (OAM) First Incorrect One Time Pin (OTP) Loop After Upgrade To 11.1.2.3

System upgraded from 11.1.2.2 to 11.1.2.3.
After upgrade all functionalities are working except one.
Expected scenario is when user enter incorrect OTP on first attempt; then the user will be redirected to login page again to enter OTP for second attempt.
This is not working, instead the user is getting an error screen and in browser "HTTP/1.1 500 Internal Server Error"
