Http 400 Bad Request - "Size of a Request Header Field Exceeds Server Limit"

Symptoms
A consumer application deployed on Weblogic 12.2.1.3 invokes an OSB Rest service. This request is processed by a vanilla Apache with the weblogic plugin 12.2.1.3.0 (not OHS) that redirects the request to one of the OSB server running in the cluster. When the issue occurs the requests aren’t redirected to the OSB servers, an HTTP error code 400 appears in the access.log of the Apache server.
In this case, the issue occurred only in production environment, restart of the Weblogic server instance is required to fix the issue.
