Activity Stream Empty Record When BLOG & WIKI Tag Deleted

Symptoms
On WebCenter Portal 12.2.1.3.0.
When a tag is added to a Wiki or Blog, the add operation is correctly mentioned in the Activity Streams task-flow (both normal and quick view). If the tag is deleted/removed, the Activity Stream will display only the time and the icon indicating the tagging operation but no other details (like blog link or the user who performed the action).
Sample steps to reproduce the behavior:
1. Create new Blog / Wiki.
2. Create Tags on it.
3. Tags should be reflected in the portal Activity stream.
4. Back to Blog / Wiki and delete tag(s).
5. Activity Stream now shows empty record(s) reflect deleted tag(s).
