"Page Not Found" Error when Trying to View Subpage Saved as Favorite

Symptoms
WebCenter Portal 12.2.1.1.
When adding a subpage as a favorite, when trying to view the favorite link for that subpage, a "Page Not Found" error is displayed.
The diagnostic log shows error with:
ADFc: No exception handler was found for an application exception.
Supplemental Detail java.lang.IllegalStateException: Cannot forward a response that is already committed
Sample steps to reproduce the behavior:
1. Create a Portal (TestPortal1). Use default/seeded/OOTB Portal template.
2. Create a Page (myPage1).
3. Create a sub Page under "myPage1" (mySubPage1).
4. View Portal. Notice "mySubPage1" is rendered.
5. Add this page to Favorites.
6. Access the newly created favorite.
An error is reported:
Page Not Found...
