Forms 12.2.1.4 fails to connect to database with ORA-12514: TNS:listener does not currently know of service requested in connect descriptor (Doc ID 2705080.1)

Symptoms

Oracle database used by Fusion Middleware Forms 12.2.1.4.0 was upgraded from 12.1.0.2.0 to 19.3.0.0.0
Afterwards, Forms will no longer connect to the database.
Middleware version is 12.2.1.4.

The error seen in the browser when calling a Forms url is:

ORA-12514:  TNS:listener does not currently know of service requested in connect descriptor

 

Changes

 The database used by the Forms application was upgraded from 12.1.0.2.0 to 19.3.0.0.0
