ODI 12c J2EE Agent Deployed in High Availability Mode Receives "ODI-1425: Agent Application Cannot Be Reached" (Doc ID 2716356.1)

Symptoms

Oracle Data Integrator (ODI) 12c J2EE Agent has been configured to use High Availability mode (HA) on Oracle HTTP Server Load Balancer (OHS), and two Managed Servers.

When either of the Managed Servers address is used in ODI Studio > Definition of the J2EE Agent, the connection test always succeeds.

When the OHS Load Balancer address is used instead, the J2EE Agent intermittently fails, and following message is received:
