Encountering Error "Application Instance already created for Resource" While Attempting To Setup GTC (Doc ID 2726622.1)

Symptoms

When attempting to setup a GTC by importing a .csv file an error is seen when attempting to save the changes.  

Error received is: An error was encountered while saving the information for generic technology connector MYGTC_IMPORT. Check the application logs for more details.
oracle.iam.provisioning.exception.DuplicateApplicationInstanceException: Application Instance already created for Resource MYGTC_IMPORT_GTC and ITResource MYGTC_IMPORT_GTC.
