How to Run the Forms Servlet on Server B Using a Static HTML on Server A (Doc ID 273567.1)

Goal

A static HTML has been created to run forms. This is working successfully on the machine where Forms is installed. What changes need to be made for this same static HTML to call successfully run Forms with the Listener Servlet if the static HTML is placed on another machine?
