Users Cannot Reset Their Password Using APEX Authentication (Doc ID 2744577.1)

Symptoms

When attempting to login after changing a user's password using APEX_UTIL.RESET_PW,
the following error occurs:

ERROR
-----------------------
"The procedure named null could not be accessed, it may not be declared or the user executing this request may not have been granted execute privilege on the procedure. Check the spelling of the procedure and check that the execute privilege has been granted to the caller"

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Create empty application.
2. Create new user and make sure "Require Change of Password on First Use" is is set.
3. Run application.
4. Enter login credentials
    -> APEX redirects to /apex/null
