HTTP 404 Error on Index.html, Index.htm and Other Files After Deploying ORDS on Weblogic (Doc ID 2751197.1)

Oracle Application Express (APEX) - Version 5.0 and later
Autonomous Database on Shared Infrastructure - Version N/A and later

Symptoms

ORDS is working in standalone mode and is running successfully against the Autonomous Database:
http://<IP>:8080/ords

The same configuration has been deployed in WLS, however when running the request, the following error occurs:
Oracle REST Data Services - 404

ORDS is up and running since the following URL presents the correct static page:
http://<IP>:port/i/20.2.0.00.20/apex_version.txt

(Where 20.2.0.00.20 represents the version of APEX on the Autonomous Database instance.)

In the WLS managed server logs (<WLSMANAGEDSERVER>.log) 404 errors similar to the following are seen for the following files associated with the ORDS deployment: index.html, index.htm, index.jsp, default.html, default.htm, default.jsp, etc.:
