Custom Attributes (Drop Down Fields) Not Working In Some Sites (Doc ID 2752409.1)

Symptoms

Basic AssetType Attributes Editor No Displayed for Custom INPUTFORM Type

After upgrade to 12c, Basic Asset Types using custom inputform for attributes are rendering as blank in the Contributor UI asset editor form:
e.g. example ADF:


In the Contributor UI, the "Example Field" editor is not displayed and in the web browser console a javascript error is seen:
e.g.
"Uncaught DOMException: Failed to execute '{operation}' on 'Document': '[name=BasicExample:name_div]' is not a valid selector.
