Symptoms

Get "System failure occurred. Contact your site administrator." in WebCenter Portal after creating a folder or navigating into a folder.  This occurred after checking "Use Screen Reader" in user preferences. 

The below error is reported in the WebCenter Portal diagnostic log when issue occurs.


Issue does not occur when "Use Screen Reader" is not checked.
