OID Upgrade Fails At Last Step Looking For OHS Templates

Symptoms
OID Upgrade Assistant (System components) fails at last step. It looks for OHS template ohs_managed_tmeplate.jar and upgrade components process fails. Here is what is displayed in the UA graphic tool:

64222: The source location does not exist for the copy definition.
64222: Correct the copy definition in the template to refer to a valid source location..
60711: Check the logs for more information..
64056: Verify template integrity or select different templates.
