Receiving Failed to Login to APEX / HTMLDB Page - You Don't Have Permission to Access /pls/apex (Doc ID 283234.1)

Symptoms

APEX (formerly HTML DB) has been installed with no errors in the installation log. When trying to access the APEX admin page after the install using the APEX URL

http://machine:port/pls/apex/apex_admin

or:
http://machine:port/pls/apex/apex


Gives the following error:

"Forbidden
You don't have permission to access /pls/apex on this server"



Other URLs within this configuration work successfully. For example:

http://machine:port brings up the Oracle HTTP Server Welcome page, and the links and tabs for Demonstrations / Key Features are also working.
