Failed to Access Forms URL After Installing Forms ---- Error 404 (Doc ID 2836076.1)

Symptoms

Accessing Forms URL after a new Forms installation fails. 
Forms installation is complete with all servers as "Running", and login to Admin Console works normally.      
However, Error 404 is returned when accessing Forms URL"http://<host>:<port>/forms/frmservlet" .

The issue persists even if restarting Forms Server.
