Starting Forms Fails with "JPS-01055: Could not create credential store instance" after Database Was Restored (Doc ID 2836244.1)

Symptoms

The databases which contains the schemas for the Fusion Middleware installation was restored.  When trying to start Forms, it fails to start, and the WLS_FORMS.log shows the following errors:

The issue can be reproduced at will with the following steps:
1. The database was restored and the schemas used by the domain were lost.
2. Forms does not start up.
