Identity Management (OIM): After Upgrade to 12.2.1.4, Some Icons on OIM Pages (ex: Modify Accounts) Do Not Render and Raise HTTP 404 Errors

Symptoms
After successful upgrade to OIM 12.2.1.4 via one-hop upgrade, it is noticed that on some pages (ex: View Account page), the icons for some of the buttons or controls do not paint correctly and appear as broken links. If it happens on a given page, it will happen both with customized versions and out of the box ones. When reviewing the log or http trace, it is obvious that the links to the images are broken, and attempts to fetch them raise HTTP 404 errors.
In the example page, the following image links fail every time:
http://oim_host/identity/icons/new_dis.png
http://oim_host/identity/icons/delete_dis.png
However, when reviewing current unmodified pages, it is clear that the images now reside under the /images rather than /icons directory:
http://oim_host/identity/images/new_dis.png
http://oim_host/identity/images/delete_dis.png
Once a user is logged into OIM, they can direct the browser to these image files (working or non-working) and either the image will display or the browser will display a 404 Not Found error.
