Intermittent Service Slowness

Symptoms
We have a service which reads database and responds the data received from database.
When we have checked access logs, some requests are taking more that 127 seconds but when we see service execution time its less than 5 seconds.
Based on observation, Starting a service itself taking 120+ seconds. We have workmanager for the service with Max thread as 4 and requests count is max 5 or 6 per minute.
