WUT-121 Error When Uploading a File to the Server (Doc ID 2936817.1)

Symptoms

After following the steps described in Document 2070183.1 to enable WebUtil in Forms 12.2.1.4.0, almost all features work except for uploading files to the server.
Upload fails with error "WUT-121" that mentions the file transfer has been forbidden by the administrator.

The configuration file includes:

Oracle user has read and write access for all directories mentioned in the configuration, but upload fails with the same error message "WUT-121."

What else could be missing for the upload to work?
