FRM-92470: Unable To Load Image Error When Attempting to Display a Custom Icon on a Form (Doc ID 2937639.1)

When attempting to open a form with a custom image, the following error occurs:

ERROR

FRM-92470: unable to load image <image name>.gif for image item

The issue is being encountered even after the steps in Method 3 of Note 960103.1 have been performed and that the image is a valid gif file.

The issue can be reproduced at will with the following steps:

1. Open a web browser.

2. Enter the URL to launch the form.
