After Upgrade from to, Login to FMW EM Console and Application Login Fails with Error "ADFSHARE-00120: Error encountered while creating the MDS Session. Application state will be reset. Please logout and log back in if problem persists" (Doc ID 2955142.1)

When trying to login to Fusion Middleware Control (/em), the login page comes up, but after entering credentials, it seems get into an endless loop and the screen is blank and "https://host:port/em/faces/helppages/errorPage.jspx" in browser address bar.

Similarly getting the same issue for the custom application, 

Following error got logged in the server logs.
