Symptoms

Upgrade readiness check was run for the upgrade of the Fusion Middleware domain from 12.2.1.2 to 12.2.1.4.
The test failed for the OHS component with the below errors in the readiness test log file:

Changes

No recent changes done to the environment. Readiness check was performed as part of the upgrade process.
