Symptoms

On a new installation of Forms 12c, the Web Form Tester is not working. Only a blank page is getting displayed when attempting to run the test form (test.fmx).

Steps to reproduce:

1. Open a web browser via a Windows client.

2. Enter the url below:
    http://<hostname>:port/forms/html/runform.htm

3. Press the Run form button.
