Forms Looking in the Old Forms Folder for .FMX Files After Upgrade (Doc ID 3013456.1)

Symptoms

Forms is looking in the old custom forms folder for the .fmx files after upgrade.

Upgraded Forms to 12c.
Old forms folder (for previous Forms version) - For example: D:\myforms
Created a new forms folder after the upgrade - For example: D:\myforms_12c. Copied all the .fmb files to the new folder and recompiled using the 12c Forms Compiler.
Also updated (using Forms MiddleWare Control) the FORMS_PATH in formsweb.cfg to include the new location.

When running the forms application, the first form, that is the login page, opens fine. However, when calling other forms from this form, it's looking for the .fmx files in the old forms folder D:\myforms.
