FRM-40735 Trigger Raised Unhandled Exception ORA-06508 When Exiting Forms

Symptoms
Forms and Reports 12.2.1.4.0 have been successfully upgraded to version 12.2.1.19.0. But the following error is being encountered when attempting to close forms. The same issue has been encountered before in and it was resolved by applying the fix to Bug 22724515. Regenerating all the libraries to create new plx files did not resolve the issue.
Steps to reproduce:
The issue can be reproduced at will with the following steps:
1. Open a terminal.
2. Launch the form using Form Standalone Launcher (FSAL)
e.g. javaw -jar frmsal.jar -url "http://<HOSTNAME>:<PORT>/forms/frmservlet?config=webutil_standaloneapp"
3. Once the form is open, try to close it using File-> Exit or pressing CTRL+q.
