APEX / HTMLDB Application Fails With Timeout Error In Mod_Plsql Log File

When attempting an action in APEX / HTMLDB, such as logging in, importing an application, etc.,   the application fails. The error log shows the following timeout error:

Application import time out apache log is as follows:
[Fri  Apr 29 11:05:52 2005] [warn] [client xxx.xxx.xxx.xxx] mod_plsql: Long running URL [/pls/htmldb/wwv_flow.accept] timed out [Fri Apr 29 11:11:07 2005] [error] [client xxx.xxx.xxx.xxx]

If Web Cache is in use, the following error is recorded in the Web Cache event log:

[09/Jun/2009:11:10:32 +0100] [req-info] [ecid: 80410028275,0] [client: xxx.xxx.xxx.xxx] [host: <hostname.domain>:7778] [url: /pls/apex/f?p=100:13:4205123654068539::NO:::]
[09/Jun/2009:11:10:32 +0100] [error 11364] [ecid: 80410028275,0] Network error response is returned.
[09/Jun/2009:11:10:32 +0100] [error 11385] [ecid: 80410028275,0] The request fails because the service is unavailable.
