Symptoms
You have created an application which uses the file upload function, similar to the sample on OTN:
However, when trying to upload a file with HTMLDB / APEX, it fails with the following error reported in the browser window.
Changes
During developement and testing, a DAD was not used and this worked just fine. However, now that the application has been changed to use a DAD the file upload functionality fails.
