Symptoms

You have created an application which uses the file upload function, similar to the sample on OTN:

However, when trying to upload a file with HTMLDB / APEX, it fails with the following error reported in the browser window.

HTTP 404 - File not found error

On inspection of the HTTP server error_log, the following error is reported.

mod_plsql: ORA-1031 Execute(Temp BLOB) ORA-01031: insufficient privileges
mod_plsql: /pls/ISR01DV/wwv_flow.accept HTTP-404 ORA-01031: insufficient privileges

Depending on which schema's are setup, you might also see following:

mod_plsql: Stale Connection due to Oracle error 942
mod_plsql: /pls/din/wwv_flow.accept ORA-942

Changes

During developement and testing, a DAD was not used and this worked just fine. However, now that the application has been changed to use a DAD the file upload functionality fails.
