Bootstrap Fails with '%userbase%: [LDAP: error code 34 - Invalid DN]' (Doc ID 333458.1)

Symptoms

Bootstrap fails with either of the following bootstrap.log errors:

ERROR: [Tue Sep 06 16:33:09 EST 2005] Initialization of the LDAP source connector - 0 to read - %userbase% failed
INFO: [Tue Sep 06 16:33:09 EST 2005] LDAP source connector - 0 search filter - null
ERROR: [Tue Sep 06 16:33:09 EST 2005] %userbase%: [LDAP: error code 34 - Invalid DN]
ERROR: [Tue Sep 06 16:33:09 EST 2005] ODIException: %userbase%: [LDAP: error code 34 - Invalid DN]

OR

ERROR: [Thu Jun 08 12:02:03 CEST 2006] users.<HOSTNAME>: [LDAP: error code 34 - 0000208F: NameErr: DSID-031001AA, Problem 2006 (BAD_NAME), data 8350, best match of: 'users.<HOSTNAME>']
