OC4J_BI_Forms Instance Keeps Restarting Repeatedly. Connection Fail With one of FRM-92102-FRM-92101-FRM-92100

Symptoms
The OC4J_BI_Forms instance is restarted very frequently, in about every 2-3 minutes. When this happens, "ipm.log" contains logs similar to below :
As a result of this problem, all running forms sessions disconnect with one of the following errors:
FRM-92100: Your connection to the Server was interrupted
FRM-92101: There was a failure in the Forms Server during startup. This could happen due to invalid configuration. Please look into the web-server log for details.
FRM-92102: A Network error has occurred.
