User Prompted Twice For Apache Server Authtype Basic Credentials When Launching Oracle Forms

This problem can occur on any platform.
Symptoms
The objective is to deploy Oracle Forms via a independent Apache Server acting as 'Proxy / SSL accelerator'
On this Apache Server, authentication has been setup so that when the Forms https URL is called for the first time, the front end Apache Server will prompt the user for authentication.
Below is a basic diagram of the topology:
Client -->
Apache Server acting as 'Proxy / SSL accelerator' (It is this Apache which has the AuthType basic, SSL configured etc)
--->
OracleAS 10g
--->
Oracle Server (Database)
However, the following sequence of events occurs:
- The user first brings up a front end web page. On this page there is a link to call the forms application
- The user is prompted to authenticate and authentication is successful
- Forms attempts to connect to the Forms Listener Servlet the front end
- Apache unexpectedly prompts for authentication again. The user should only need to authenticate once
