Rep-52251: Cannot Get Output of Job Id###, when Calling Reports from Forms

Symptoms
Running reports under HA environment having two or more clustered and load balanced midtiers.
Intermittently it fails with following error:
REP-52251: Cannot get output of job ID 54 you requested on < date and Time >
You have followed below notes, but still the issue persists:
<Note:333981.1> Migration to 10.1.2.0.2 with Report Server Clustering
<Note:358436.1> Maintaining 6i Run_Report_Object Code in 10.1.2 Without Employing Clustering/HA
Changes
Previously It was working and now it is Migrated to HA Environment having Load-balancer with/without webcache.
Each midtier under Load-balancer are using default report server.
