Symptoms

JSP pages on all OC4J applications are returning sporadic 404 errors that are shown on the browser, as well as on the HTTP Server logs.
Another symptom that is related to this issue is that at times OHS will consume a lot of CPU (nearly 100%), and then restart.
We have confirmed the restart is related to the HTTP-404 errors by correlating the restart times (logs monitoring the process identifier, pid) with the HTTP-404 errors in the error_log files.

instance_oc4j log file is showing the following errors:

mod_oc4j: There is no oc4j process (for destination: oc4j_isc) available to service request.
mod_oc4j: Failed to find a failover oc4j process for session request for destination: oc4j_isc (no island or jgroup).
