Symptoms

Error "Exception in thread 'main' java.lang.IndexOutOfBoundsException" when trying to convert
test.fmb trace file to xml/html through the Xlate utility in Forms 10.1.2.0.2.

-- Steps To Reproduce:
1) Generate trace for test.fmb by running with 'record=forms&tracegroup=debug' in the URL.

2) Generate the xml/html file from the trace file e.g :

++ Set the required environment variables
