Unable to Login As Admin In /Pls/Apex/Apex_admin (Doc ID 444333.1)

Symptoms

When entering the URL http://host:port/pls/apex/apex_admin the admin page comes up, but it is not possible to login to Apex using the Admin password.

In addition, when the page comes up, it is missing icons. The HTTP Server access_log shows the following for the /pls/apex request:
