Intermittent FRM-93000 When Pressing Radio Button On The Form After Upgrade TO 10.1.2.2 (Doc ID 556809.1)

Symptoms

After upgrading Forms there is occuring now intermittent the following error with specific Forms when pressing a Radio Button on the Form:

ERROR
-----------------------
FRM-93000 : Unexpected internal error -->
- Forms session <93> aborted: unable to communicate with runtime process.
- Forms session <93> exception stack trace:
java.io.IOException: FRM-93000: Unexpected internal error.
Details : No HTTP headers received from runform
...
In trace file you can see also:
<ABORT_FORMS_DIED Name="Forms encountered unexpected signal 6">

The issue can be reproduced at will with the following steps:
-------------------------------------------------------------------------
1. Run the specific 10.1.2.2 Form and
2. Press a Radio Button on the Form

Changes

Upgrade from Forms 9.0.4.2 to Forms 10.1.2.2
