Provisioning Console Does Not Appear To Work As Stated In IDM Integration Guide

OID Provisioning with E-Business Suite is configured.
According to the Oracle® Identity Management Integration Guide10g (10.1.4.0.1), Provisioning and Deprovisioning Users with the Provisioning Console section, you can remove a user's provision (deprovision) and add a user's provision from the General Provisioning window in the Self Service Console. However after logging into OIDDAS and following steps 1-4, the General Provisioning window does not display.
After selecting the user to provision or deprovision, then click Edit button, the next button does not appear.
