A message from Commit Server log states:
Failed to commit batch AIS0000000015954. Error Number: 40008, Error: The following error was reported by ReleaseDocument. Error Number: 21006 Error Description: Failed to add page \\SERVER\SHARE\Batches\001\000006EK\000006EK.2. Error Number: -2147418113 Error Description: Catastrophic failureAccess Violation(0xc0000005) class ATL::CComObject<class CDocumentManager>::CreateDocument.
