The purpose of this document is to help diagnose situations encountering the following error when using Run Report Object (RRO) to call Reports from Forms:

FRM-41213: Unable to connect to the Report server %s.

It provides a diagnostic FMB and explains how to use it.

This note is related to Forms / Reports 11g only . For previous versions , refer to the note :

This note applies in situations when  encountering FRM-41213 when using  (RRO) to call a Report.
If the cause of the problem cannot be found with the traces generated,
please create a Service Request and provide the two trace files generated to Oracle Support.
