During 10.1.4.0.1, DCM Configuration Assistant Fails with "LDAP; error code 50 - Insufficient Access"

Symptoms
There are two nodes and the 10.2.0.4 database and Metadata Repository Configuration Assistant (MRCA) were installed on both nodes successfully. For the first node, the Identity Management Installation Installer was run with options 1) Identity management 2) Oracle Internet Directory and 3) Oracle Directory Integration without any problems. For a second node, the Identity Management Installation Installer was run with options 1) Identity management 2) Oracle Internet Directory, and 3) Oracle Directory Integration and 4) High Availability with LDAP two way replication, but this second node installation fails on the DCM Configuration Assistant.
The install actions log file shows the following errors:
