Symptoms

You are using the FileStoreProvider component, and have the storage rule set up as a webless storage.  When using the GET_DYNAMIC_CONVERSION service with the parameters of dDocName and LatestReleased, the dynamic conversion returned is not always the latest revision of the content item.
