IDM 10.1.4.0.1 Installation DBCA fails with ORA-01041 Error

Symptoms
Installing Oracle Identity Management Infrastructure and Oracle Identity Federation (10.1.4.0.1) fails during the Database Configuration Assistant phase (DBCA). The process gets to about 2% and then fails with the following error:
ora-01041 :internal error: hostdev extention does not exist
The following error may be seen in the rdbms logs:
$ORACLE_HOME/rdbms/log/
ORA-07445: exception encountered: core dump [kcbbnwa()+360] [SIGFPE] [Integer divide by zero] [0x100EEC428] [] []
