OID 10g bulkload Fails to Generate or Load Data

Symptoms
Bulkload does not generate or load data, it seems to run without errors but no entries are loaded.
All *.dat files in $ORACLE_HOME/ldap/load directory are 0 bytes.
Command output:
Other symptoms:
oiddiag utility reports a missing index.
catalog utility cannot delete the index, reporting that it is not indexed.
bulkload index reports error in getting attribute syntax.
