A Call Submitted With a New Trouble Code Is Not Created Correctly, Code Column Is Blank and Power Type Is OUT when It Should Be ON

Symptoms
There are currently five choices under the 'Emergency' category in the Trouble Call Entry screen including N/A. Six more choices were added for a total of 11 choices.
All 11 items appear in the Emergency list and any one of them can be selected however, only the first ten work properly. More specifically, the trouble codes with CODE_NUM in the range of 0 to 9 (in the SRS_TROUBLE_CODES table) work properly.
When the last item in the list (CODE_NUM=10) is selected, and a call submitted, the event is created with the following characteristics:
1) The 'Code' column in the Work Agenda is blank (for example, it should display 'Elec Cont' for example).
2) The power type is automatically set to 'OUT' even when selecting the power type to be 'On'.
3) The event is not treated as if it is an emergency (no 'E' in the E column).
