Symptoms

Error can occur in the Rules Language on a FOR EACH IN LIST statement or a LISTVALUE statement, or in the Table-Column list edit window if values are provided to the list instead of identifiers.

The full error message is: 

"A column with name 'X' already exists in resultset. Missing prefix in one of metadata references for the 'TABLE1' table may lead to this error."

The error message can also be shown as:

"An item with the same key has already been added."
