Facing problem in creating new account from BRM 7.4 Selfcare Manager Application.?

1. Added a link 'Create New Account' in view_balance.jsp. After login into Selfcare Manager when click on link it navigates to CreateForm1.jsp page.
2. Select the plan and click on continue. Then it navigates to CreateFrom2.jsp page.
3. After filling out all fields in CreatePage2.jsp page and clicking on Continue.
4. Getting this Execption
"org.apache.jasper.JasperException: java.rmi.RemoteException: error.general; nested exception is:
ERR_NOT_FOUND"
