How do you add a custom / user defined dimension (leaf) to Oracle Financial Services Analytical Applications (OFSAA)?

Note: This document does not apply to the Oracle Insurance Allocation Manager for Enterprise Profitability application.  This application has its own Dimension registration program called LEAF_REGISTRATION_PFT_EXT.
