Symptoms
The Mobile Workforce Management (MWM) Dispatcher Workstation (DW) Gantt Chart within the application will not load, leaves a blank window and gives the message 'Create Report Form Failed'. When closing the 'Create Report Form Failed' error dialog, the MWM application crashes. A reboot of the workstation has not corrected the problem. The application on this workstation has worked successfully for some time and other workstations are able to load the Gantt without a problem.
